Fostering a Future Service Dog for On Command K-9 Academy

Please note:

On Command K-9 Academy will be responsible for vaccinations, spay and neuters, and any sickness or injury not deemed to be negligence on the part of the puppy raiser.

You are acknowledged that the information you have provided is true and correct.

You are acknowledging that you are willing to volunteer your time to raise a service dog puppy in your home for a minimal of 3-14 months of age.

You are aware that any purchase of food, training equipment, crates, leashes, collars, etc. will be at your own expense.  You will retain these when the fostering period is over.

You will not be reimbursed by On Command K-9 Acdemy for any purchased item for this puppy unless it has been approved by On Command K-9 Academy prior to the purchase.

You acknowledge in advance that you are volunteering your time to give back to the community that you live in.  To help give a disabled person a better quality of life, using a Service Dog.

I also acknowledge that On Command K-9 Academy has sole ownership of the puppy that I am volunteering to raise.

I acknowledge in advance that I will relinquish the puppy upon request of On Command K-9 Academy back to them.
